Observation report or clinical Information in CDA standard with MFER (medical waveform format encoding rules)
CDA is a good standard for clinical information as persistent object. And HL7/V2 provides the observation report standard for laboratory results, however the standard has not become popular for physiological examination result. The reason is the physiological examination absolutely requires a medical waveform itself such as ECG, EEG, spirometry etc. However, a description of medical waveform is necessary not only expertise, but also complicated and specialized representation. These requirements are quite difficult for medical information system venders, and medical manufacture cooperation is absolutely required. Most medical equipment has many unique specifications for each, but current standards such as SCP-ECG are not able to describe these unique features. Therefore, such kind of standard is not widely used even if these standards were developed a decade ago. MFER is designed to aim decreasing sacrifice the unique features and to minimize the implementation burden. These features are already demonstrated by many manufacturers. The medical waveform must be utilized in clinical research, therefore the simplicity and easy description makes MFER a very useful standard. I presented ECG report in CDA with MFER in last year’s international affiliate meeting in Taiwan. The physiological report and clinical representation with CDA and MFER is useful in physiological information not only ECG but also other clinical summaries, reports, IHE and so on.
